  • Step #2 - Engagement & Contracting

    If it feels like a good fit, we’ll send a clear service agreement outlining scope, fees, confidentiality/HIPAA, and communication preferences.

    Once you sign and pay the initial fee or retainer, we get started.

  • Step #3 - Comprehensive Assessment & Care Map Creation

    We review your medical history, provider list, medications, and insurance details.
    Then we identify any gaps, specialist needs, or transitions and deliver a personalized Care Map / Journey Plan that shows your next steps.

  • Step #4 - Implementation & Coordination

    We put your Care Map into action — scheduling referrals and appointments, transferring records, prepping for visits, and providing summaries afterward.

    You’ll have a single point of contact who keeps everything organized and connected.

  • Step #5 - Ongoing Monitoring & Transition Support

    We stay involved, checking in regularly, reviewing updates (labs, imaging, progress), and helping you or your loved one through major transitions — like moving from pediatric to adult care or adjusting to a new provider or insurance.

  • Step #6 - Review, Evaluation & Maintenance or Handoff

    Every 6–12 months, we review your progress and decide what’s next: continue full coordination, shift to maintenance mode, or transition things back to you with a detailed toolkit and summary.

  • Step #7 - Closure or Continued Partnership

    When you’re ready to manage things independently, we’ll provide a final summary and on-call support as needed. Or, if you’d like us to stay on as your long-term advocacy partner, we’ll keep working together for ongoing support and peace of mind.
