About My Connected Patient

Meet Anne Wienke, RN

With over 33 years as a Registered Nurse and more than 14 years in care navigation, Anne helps patients and families find clarity, confidence, and connection in the healthcare system.

Anne Wienke is a dedicated nurse and patient advocate with over three decades of experience in healthcare. Throughout her career, she has worked in a variety of capacities, tirelessly advocating for patients and their families. Committed to staying current in practice, Anne helps individuals navigate the often complex and dynamic healthcare environment with confidence and clarity.

Inspired to care for patients from an early age, Anne brings both professional expertise and deeply personal insight to her work. As the mother of two daughters who have faced rare and complex medical conditions, she understands firsthand the challenges families encounter when coordinating care across multiple specialties and systems.

Outside of her professional work, Anne enjoys running marathons and spending time with her family—activities that ground her, inspire her, and fuel her commitment to the people she serves.

How I Can Help

Guiding Rare Disease Families

I guide rare disease families by helping them navigate complex medical systems, connecting them to the right resources, specialists, resources, and support networks, rooted in years of hands-on experiences. As an Ehlers Danlos ECHO advocacy trained patient advocate and as  someone who has presented at the rare Disease Council, I offer families a trusted partner in health care coordination.

Coordinating Complex Transitions

I coordinate complex transitions across the lifespan, guiding adolescents from pediatric to adult care services and supporting adults as they navigate retirement and long term care resources.

Advocating Beyond the Clinic

I help families navigate complex medical and community systems, secure funding and benefits, and access the services that support real life needs.

“I believe patients and families deserve clarity, confidence, and compassion in every step of their care. Let me help guide and support you through every step of your care.
