My Connected Patient is a patient advocacy service committed to accessibility, inclusion, and equity for individuals with disabilities, chronic illness, complex medical needs, and neurodivergent differences.

Advocacy should never create barriers. We understand that navigating healthcare, insurance, education, and support systems can be overwhelming—especially for individuals living with conditions such as EDS, POTS, MCAS, ASD, and other complex or invisible disabilities. Our advocacy services are designed to meet people where they are.

We strive to provide:

  • Clear, plain-language explanations of medical, insurance, and systems-based information

  • Flexible advocacy support tailored to individual access needs

  • Remote and tele-advocacy options when in-person support is not accessible

  • Support for executive functioning, sensory, cognitive, and energy limitations

  • Assistance communicating with providers, schools, employers, and agencies when needed
